Israel Nachfolger, RN, ASN, BS, NR-EMTP

Owner & CEO

Israel brings a unique mix of clinical and operational experience to Pinnacle, having worked in healthcare as both a practitioner and in various administration roles for over a decade. In addition to his role as CEO, Israel continues to practice as a Registered Nurse and volunteer Paramedic while managing multiple Skilled Nursing Facilities on the East Coast.

Israel lives in the foothills of the Ramapo Mountains in the Hudson Valley in Monsey, NY with his wife, Miriam, and 6 children. An avid lover of music, Israel enjoys honing his skills as a musician at the piano and practicing his passion as a recording engineer in his free time.

Jeff Ketchum, MS, MLA


Jeff has a well-rounded background of experience in the continuum of care for seniors in both the private senior living communities and skilled and long term care facilities. Jeff entered the healthcare field as the Executive Director of The Highlands Retirement Community. While there he obtained his Masters in the Management of Aging Services from the University of Maryland at Baltimore Country. He recently acquired his Nursing Home Administrator’s license and joined Pinnacle Health and Rehabilitation at South Portland in May of 2018. He brings a tremendous amount of managerial skills and believes in raising the bar on quality, standards, and results.

Jeff and his wife Lori enjoy spending time with their son Lucas and their four pugs. As an outdoorsman, Jeff enjoys camping, hunting, fishing, skiing, and golfing.

Gail Hauser, RN, DNS-CT

Director of Nursing Services

Gail’s nursing journey has spanned 35 years as a C.N.A, LPN and RN providing care ranging from newborns to end-of-life services. Roles such as Corporate Director of Clinical Operations & DNS have enabled her to become a healthcare change agent.

She has championed many quality improvement programs with the Senior Living Collaborative and various Maine Health Care Association committees. These initiatives included Electronic records, developing the Nurse Manager Certificate program, Safe Mobility work group, SNF to ED transitions and a Bronze award winning Wound Prevention team. She enjoys being a servaint leader and having staff reach their goals as well.

Gail and her husband Larry enjoy a life sharing their boat on Sebago Lake with their grown family and 9 grandchildren.

Judy Noel

Activities Director

Judy has been providing the residents of our facility with exciting and fulfilling life enrichment activities since 1993. With almost thirty years of experience creating meaningful programming, Judy keeps our residents active and engaged throughout the day.

Judy resides in Kennebunk with her husband of over forty years and considers herself blessed to have two married sons and four grandchildren who live near-by. In her free time, she enjoys going up to camp in Oakland, reading, and spending time with her family and her two lovely golden retrievers.

Caroline Brockway, OTR/L

Director of Rehabilitation Services

Caroline joined the Rehab team at Pinnacle as a per diem Occupational Therapist in May of 2022. In August 2022, she welcomed the opportunity to lead the department as Director of Rehabilitation Services. Caroline received her undergraduate degree in the Bachelor of Arts program at Connecticut College in 2010. After several years in the workforce, Caroline honed in on her passion for working with older adults and went back to school to receive her Master’s of Science in Occupational Therapy from the University of New England in 2019.

Caroline lives in South Portland with her husband Kevin and daughter Morgan. She enjoys spending free time hiking, swimming, day tripping around Maine, and playing with her daughter.

Mary E. Murphy, LSW

Director of Social Services & Admissions

Mary has an incredible forty year history at our facility having worked at this location since 1978! She considers herself blessed to have been a part of our wonderful community for such a long time. One of her favorite quotes is by Marc Anthony, “If you do what you love, you’ll never work a day in your life”. This truly sums up Mary’s ethic as the voice of Pinnacle South Portland. As a tireless advocate for our residents, Mary guides families and collaborates with others in the health care continuum to ensure the best of care for those residing at our facility.

Mary received her bachelor’s Degree from UMF and her Master’s Degree in Counseling from USM. She and her husband Bill live in Gorham and are proud to have two children and one granddaughter.

Scott Parker

Environmental Services Director

Scott is a former U.S. Marine with a background in security and has a degree in Industrial Electricity.

He has been with Pinnacle for 4 years in the Maintenance Department. He enjoys the comradery with the residents even going so far as playing our local Santa.

Scott resides in Lyman Maine with his family, he enjoys cooking, fishing, and spending time with his grandchildren and dogs in his free time.

Tina Allen, CDM, CFPP

Food Service Director

Tina began her journey at our facility back in October 1988 as the Food Service Director/Certified Dietary Manager. With over 30 years of experience catering to our residents, Tina has developed a unique ability to know exactly what each and every resident prefers for their meals. Our residents enjoy being part of the culinary experience at Pinnacle and frequently share their own recipes with Tina and her team which are implemented into our regular menus.

Tina and her husband Tom reside in Cape Elizabeth and have two children, one cat and one dog. Their greatest joy is their grandson who was just born in 2020. In her free time, Tina enjoys cooking, camping, making crafts, and spending time with the family.

Kelly Parker

Business Office Manager

Kelly has over 25 years of experience with the nursing home, where she has worked in many different roles. For the last ten years she has been working in the Business Office doing Payroll and Benefits. She loves to be able to help the residents, families and staff.

Kelly resides in Lyman, Maine with her husband and their dogs and cats. She loves animals and spending time on the lake with family and friends. She has a daughter and family nearby and a son and family that live in Texas. Her greatest joy is being able to spoil her four grandchildren.
